The following El Dorado Hills Little League forms/documents are available

Available Forms:
Medical
Release Form
Now available . All players
must provide their manager with a completed and signed medical release
form each year. No player is allowed to practice with a team or play
in a game if they haven’t submitted a signed form.
Manager Evaluation
Please help us to make Little league a
better experience by filling out this manager evaluation form.
Injury
Tracking Form
Managers, fill out this form and submit it to the league when a player
is injured during a game or practice.
Volunteer
Application Form
Little League policy requires that all volunteers (managers, coaches,
team parents, league officials) complete and sign a volunteer application
form. This form needs to be completed each year.
Parents
Code of Conduct
All volunteers are required to read and sign the Parents Code of Conduct.
Forms are to be attached to the Volunteer Application Form when submitted
to the league.
Sponsor Form
Sponsor forms must completed by organizations or individualswho want
to sponsor an EDHLL team. In addition to indicating the level
of sponsorship, the form provides the necessary contact and background
information on the sponsor.
